What is Support Coordination?
Support coordination is a capacity-building support that helps you make the best use of the supports in your NDIS plan. A support coordinator helps you understand and use your plan to reach your self-determined goals, connects you with NDIS providers, community, mainstream and government services, and builds your ability to use and coordinate your supports yourself.
Three levels of support coordination
Support Connection (Level 1) builds your ability to connect with informal, community and funded supports. Support Coordination (Level 2) helps you build the skills to understand and use your plan, keep relationships going, manage service delivery and live more independently. Specialist Support Coordination (Level 3) is a higher level of support for people whose situations are more complex, delivered by a specialist who helps you manage challenges in your support environment and keep services consistent. Australian Carers is registered for all three.
